At Intuitive Choice Physical Therapy & Wellness, we provide one-on-one, in-home physical therapy in Peachtree City and Fayette County, Georgia, using an insurance model that prioritizes quality, convenience, and results—not red tape.

What Insurance Plans Do We Accept?

We work with the following insurance options for in-home physical therapy services:

  • Medicare
  • Medicare Advantage PPO plans
  • Commercial PPO plans

🚫 We are unable to bill HMO plans. If you have an HMO, our self-pay rates would apply.

Three Ways to Work With Us

1. Medicare + Supplemental Plan

Often fully covered

If you have Medicare Part B with a supplemental plan:

  • Physical therapy is often covered at 100% after your annual deductible

  • The 2026 Medicare Part B deductible is $283

  • Once met, most clients have little to no out-of-pocket cost

This option works well for medically necessary in-home physical therapy.

2. Medicare Advantage PPO Plans

Deductible + small copay may apply

Medicare Advantage PPO plans vary by carrier but commonly include:

  • A deductible (what you pay before coverage begins)

  • A copay per visit, often $15–$40, depending on your plan

We help verify benefits whenever possible so you understand your costs before starting care.

3. Self-Pay (Private Pay)

Evaluation: $200
Follow-Up Visits: $160

Self-pay is commonly used by:

  • Clients with large deductibles

  • Those choosing not to file insurance

  • Clients submitting out-of-network superbills

  • Medicare beneficiaries currently receiving home health or hospice services

  • Anyone seeking maintenance, wellness, or prevention-based care after discharge

  • Clients with HMO plans, which we cannot bill

Good Faith Estimates for Self-Pay Clients

If you choose self-pay, we believe in complete transparency.

In accordance with federal law, self-pay clients are provided with a Good Faith Estimate before beginning care. This estimate outlines the expected cost of services so you can make informed decisions and avoid surprises.

Your Good Faith Estimate is based on:

  • Your evaluation findings

  • The anticipated number of visits

  • The type of services recommended

If your care needs change, we communicate openly and update your estimate as needed.
